根據「博士生資格考核及學位考試實施要點」第二點第三項第一款

1.博士生在入學第一學期前須選定指導教授，至少一人應為本院專任助理教授以上教師，惟博士生之配偶或三親等內之血親、姻親，不得擔任其指導教授，並提交經指導教授同意簽章後之「本校研究生指導教授提報申請單」至本院備查。

請於入學後第一學期（內），將本單經指導教授簽可後送各系（所）彙整備查。
